Over the past few years, GLP-1 medications like Ozempic, Wegovy, and Mounjaro have taken center stage as the “miracle” solution for quick, effortless weight loss.
But a brand-new meta-review published in the British Medical Journal (BMJ 2026;392:e085304) is raising serious questions, especially for midlife women who are already navigating hormone shifts, metabolic changes, and years of weight-loss frustration.
This landmark review included 37 studies and more than 9,000 participants, making it the most comprehensive evaluation of GLP-1 discontinuation outcomes to date.
And the results are… shocking.
Across all three analytical models used in the review (yes, the researchers triple-checked the data), the same conclusion emerged:
After stopping GLP-1 medications, weight regain happens four times faster than expected.
Even more concerning?
The authors found no evidence of sustained benefit at 1.7 years after stopping the medication.
No benefit.
Not even a small residual effect.
This means that the vast majority of people who lose weight on these medications regain it, and quickly, unless they stay on them indefinitely.
One common defense of GLP-1s is that the “newer” generation of medications (like semaglutide or tirzepatide) produces more weight loss, so the long-term outcomes must be better.
But this review found something surprising:
Yes, the same regain trajectory, even after dramatic weight loss.
The reason this matters?
Most marketing and media coverage paints newer GLP-1s as fundamentally different or more sustainable.
But according to this review, the rebound risk appears unchanged.
The researchers addressed this.
Although the umbrella analysis included all weight-management medications (such as metformin and other incretin mimetics), the study also:
And they did.
The rapid weight-regain signal showed up every single time.
Women in perimenopause and menopause already face:
GLP-1 medications can initially mask these shifts by suppressing appetite, but they do not fix the underlying metabolic issues driving weight gain.
This is why rebound might feel even more intense for midlife women.
When you stop the medication:
It becomes the perfect storm for rapid regain.
For years, clinicians and researchers have whispered about the “Weight Rebound Effect.”
But this BMJ review brings it into the spotlight:
Whether it’s old GLP-1s, new GLP-1s, or any appetite-suppressing medication, once you stop, the biology you were trying to bypass comes right back.
This doesn’t mean these medications can never be helpful. But it does mean they must be paired with:
Otherwise the rebound isn’t a “risk” - It’s the expectation.

Here is what truly moves the needle long-term:
Build muscle, balance blood sugar, eat enough protein, and stabilize cortisol.
Support estrogen, progesterone, thyroid health, and adrenal function.
You cannot lose weight in fight-or-flight mode. Breathwork, light exposure, sleep, and gentle movement matter.
Your microbiome directly influences cravings, satiety, immunity, detox, and hormone balance.
No more all-or-nothing dieting. Lasting results come from consistency, not perfection.
Midlife weight loss is not just about knowing what to do, it’s about having someone walk beside you so you actually do it.
This isn’t about fear. This is about empowerment.
The new BMJ review confirms what many clinicians (and many of you) have already experienced:
GLP-1 medications may help you lose weight quickly, but they do not help you keep it off.
You deserve a better, more sustainable approach, one that works with your body, not against it.
